KAMBO WAIVER

Please take your time and read this agreement very carefully. Sign the document only after you have read and understood everything. If you have any questions about the agreement, you may contact Dr Cat Meyer. Thank you for your attention to this matter.

By submitting this waiver, I agree to the following:

  • I apply for and consent to a session of Kambo therapy.
  • I understand that the general process and results of Kambo therapy can vary for each individual and that specific results cannot be guaranteed.
  • I affirm that I have or will notify my practitioner of all known medical conditions and injuries (and any changes occurring) before my first Kambo therapy session.
  • I understand that the services offered are not a substitute for traditional medical care nor is this entering into a psychotherapeutic working relationship.
  • I understand that the practitioner does not treat, prescribe for, or diagnose any illness, disease, or any other physical or mental disorder, injury, or condition. Nothing said or done by the practitioner should be construed to be such.
  • I further understand that the practitioner is not attempting to practice medicine, psychology, or any other profession requiring a license.
  • I agree to pay for any appointments that I miss or do not cancel with 48 hours advance notice. 

 

Upon signing this, I give up my right to sue Dr Cat Meyer or anyone associated with Dr Cat Meyer for injuries or losses I may suffer while undergoing any treatment or while participating in any ceremony with Dr Cat Meyer. 

 

In consideration of, and in part payment for the right to partake in a treatment/ceremony with Dr Cat Meyer, I agree to defend, protect, INDEMNIFY, and hold harmless Dr Cat Meyer, her friends, family, + training agency from and against any and all claims, suits, actions at law or in equity, for damages or other relief and against any liability of any nature, together with attorneys' fees and costs incurred, that may arise out of my decision to partake in a treatment/ceremony with Dr Cat Meyer, INCLUDING BUT NOT LIMITED TO INJURIES ARISING FROM MY VIOLATION OF HER RULES, RECOMMENDATIONS, AND CONTRADICTIONS FOR KAMBO. I agree to pay the reasonable attorneys' fees and all other costs of all parties if I bring a suit for injuries suffered from Dr Cat Meyer and that action is unsuccessful, in whole or in part.
